Please use the revised Intake Booklet during the 2026 tax season

Steps to download and complete the Intake Booklet

1 - Click on the Pop-Out icon (upper right hand corner) to open document in a new screen

2 - Select the Print icon OR download the document to your computer

3 - Fill out the fillable PDF document

4 - Print the document (double sided) and save to your computer (if you want to keep an electronic copy)

4 - Bring the completed Intake Booklet to your tax appointment.
